Are you looking to register your child in SAC? Well you have come to the right place! Here you will find all of the files and information needed for registration. You will find download links on the right for all of the necessary files. If you’re having problems downloading and/or viewing the files, you probably need to download and install Adobe Reader on your computer. Below is some information about each form.
Each account automatically gets billed the Friday prior to the week. The amount you will be billed every week depends on what schedule of enrollment you choose on this form. Multiple children’s information can be entered on the same enrollment form if they attend the same school. If you have multiple children that attended separate schools, then you will need to fill out an enrollment form for each site.
Emergency Information Sheet
This not only has space available for information about you and your child but also has an area to list people to whom your child may be released. It is very important that this sheet is filled out in its entirety and kept up-to-date. In the event of an emergency, if we first cannot get in touch with the mother and/or father (or legal guardian), we will then attempt to contact people on the pickup list.
Here you will find policies and procedures, rates, holidays, and other important information about our program. In order to enroll, parents must sign the appropriate place on the emergency information sheet agreeing to abide by the guidelines as stated in the Parent Handbook.
This is our all-in-one permission slip form for applying bug spray and sunscreen to your children as needed, as well as allowing us to take pictures.
Sibling Discount Form
If you have more than one child that will be attending our program, you will need to download and fill out this form. You will still receive a sibling discount if your children attend different schools.
Employee Discount Form
Employees of the Putnam County Board of Education receive a 20% discount on tuition. Print and fill out this form if you are employed with Putnam County Schools.
Along with the necessary forms, you will also need to bring a check or money order for the amount of the first week’s tuition and a one time $10.00 registration fee per child. All of this needs to be turned into the SAC Site Director at the school you wish to attend. If you need directions or wish to contact your director, head on over to our Contact page.
